Discharge summary transcription reports

These are reports created when the patient is discharged from her/his stay in the hospital, giving a summary of the reason for stay in hospital, the procedures and conditions that the patient went through when in the hospital, the medications to be taken after discharge, discharge plans and follow up notes. We do this for most hospitals as the primary report along with H&P and Operative reports.
